Once the parish registration form (below) is completed, please mail or drop by the Parish Office. 
Our office hours are Monday-Friday, 8:30a.m. - 4:00p.m.  If you prefer you may visit our parish office and pick up a hard copy.  Either way, we will begin to process your membership as soon as the information provided has been received.
